Liability Waiver Form

• I acknowledge that participating in activities sanctioned by Ghetto Fitness involves risk of injuries, loss, or damages on Ghetto Fitness premises.
• I assume all risks associated with my participation including, without limitation to, the risk of any negligence or recklessness by other participants. I understand that there may be risks involved which are not known to me or Ghetto Fitness at this time or at the time of activities in which I may participate, and agree to assume such unforeseeable risks.
• I agree to indemnify, defend and hold harmless to Ghetto Fitness and their employees, agents and volunteers from and against any claims, causes of action, damage, judgments, liabilities, fees (including legal fees), costs and expenses incurred by Ghetto Fitness as a result of my unlawful actions or failure to act during any activity.
• I confirm that I am in good health and have no physical conditions that affect my ability to participate in any sanctioned activity conducted by Ghetto Fitness and have not been advised otherwise by a medical practitioner. 
• Ghetto Fitness will not be liable for any loss or theft of any personal property.
• Ghetto Fitness reserve the rights to ask members not to participate in the class if we deem them unfit (e.g If the member suffers from common cold, cough, or any illness that is airborne).
• Ghetto Fitness reserves the right to use images taken from any training sessions for social media and marketing purposes. 

Terms & Conditions

1. Members can make their class booking via phone, in person at the studio reception, or via the Online Booking System.
2. Members who are on any unlimited class packages are reminded that they will be liable for a penalty fine of $15 if they are absent from classes they booked.
3. Members who are on any per session packages are reminded that they will forfeit 1 class or its equivalent credit if they are absent from classes they booked.
4. Members are not allowed to cancel classes that have been booked on the day itself.
5. The company reserves the right to cancel, change the class type or instructor without prior notice.
6. Personal training sessions that are not rescheduled or cancelled 24 hours prior to the session itself will result in forfeiture of 1 session or its equivalent credit.
7. Extension of package due to medical reasons is allowed if supported by medical certificate or report.
8. Members arriving late will receive the remaining scheduled session time.
9. Package fees are non-refundable.
10. Any session not used after the expiration date will be forfeited.
